Young People Working Group

 

1. Purpose

The purpose of the A2Z Care Coordination Panel will be to coordinate multi-agency responses to identify and overcome systemic barriers to sustainable housing outcomes for people who are homeless, or at risk of homelessness.

The panel will achieve these outcomes through the collaborative use of supports and resources to reach agreed recommendations and actions. These recommendations and actions will remain the responsibility of the referring agency, however the Regional Care Coordinator will maintain communication to support this process. The panel will review progress of these recommendations and actions and adapt as needed.

2. Principals

The A2Z Care Coordination Panel will be able to develop a process that is consistent with agreed high-level principles. These are intended to bring together and build on the skills and strengths of the multi-disciplinary group to achieve the care coordination purpose.

The A2Z Care Coordination Panel principles are:

  • Person-centred practice – Maintaining a core focus on the best interests of people at the centre of decisions regarding their support and housing needs. 

  • Housing First – Working within a consistent framework that looks to provide safe and stable housing as a first priority in supporting people.

  • Collaboration – Participating in a spirit of cooperation to create effective and efficient working relationships that benefit the people recommended to the panel.

  • Transparency – Working together across organisations in sharing information together to achieve outcomes and collect data to communicate successes.

  • Place-based – A structure that is owned by the Gold Coast region to ensure the group meets the needs of their community.

  • Continuous improvement – Members review the Care Coordination Panel processes and Terms of Reference to ensure that they continue to meet its purpose.

 

3. Goals

The A2Z Care Coordination Panel goals are:

  • Identify, document, and respond to systemic barriers and/or gaps in service delivery for people experiencing, or at risk of, homelessness.

  • Work collaboratively across organisations to support people experiencing, or at risk of, homelessness.

  • Connect across organisations and improve, or develop, partnerships to enhance service delivery outcomes.

  • Bring together housing and support services that are tailored to suit the identified needs of people presented to the panel.

  • Explore proactive engagement strategies with people that face barriers to engaging with support services.

  • Document and showcase the efforts and achievements of the housing and homelessness sector in the Gold Coast LGA to the broader community.

  • Continue to build an evidence base of supporting people to access housing outcomes and sustain tenancies.

  • Identify opportunities to support the housing and homelessness workforce including training and development programs that can be delivered across organisations.
